Euthanasia and assisted suicide have become highly relevant and controversial issues in contemporary medical ethics. These ethical dilemmas arise in the context of the increasing importance of patient autonomy and the need to balance this principle with other ethical considerations, such as non-maleficence and justice. The evolution of legislations and social attitudes towards euthanasia reflects the complexity of these issues in different cultural contexts. The present study aims to analyse ethical dilemmas in medical decision-making related to euthanasia, assessing the philosophical, legal and cultural perspectives that influence this phenomenon. A systematic review of the scientific literature published between 2017-2024 was conducted, using recognised academic databases. Relevant articles addressing the ethical, legal and practical aspects of euthanasia and assisted suicide were selected. The results highlight the diversity of opinions and approaches on euthanasia, emphasising the need for a clear legal framework and the importance of palliative care as an alternative. Furthermore, the ethical implications in the doctor-patient relationship, and the influence of cultural factors on the perception of euthanasia are discussed. The analysis of the ethical dilemmas surrounding euthanasia reveals the complexity of this issue in modern medicine. A continuous and respectful dialogue is required that considers the multiple dimensions of autonomy, dignity and human suffering, ensuring informed and compassionate decisions at the end of life.
